We ask for your UC ID number (which can be found on your pay stub) in anticipation of eventual integration of the A-Z list into the campus directory system. Having a unique identifier for each site owner will help assure that all sites submitted are affiliated with UCSF.
The UC ID number will also help to gather basic directory information. We do not intend to collect any information other than name, work phone number, email address, and department. The UC ID number will not be visible to anyone other than the administrator of this web site.
